• Print this form, select the options you need us to perform, sign at the bottom and bring in with your pet on the day of surgery.

  Standard anesthesia requirements:

  Make appointment with the clinic to select the day of the procedure.

  Do not feed or water your pet after midnight the night before surgery. No food or water or treats of any kind for at least 8 hours before surgery.  This makes the anesthesia safer.
